What to expect at your first visit

The paperwork that you will need for your first visit is right here on the website. If you’re coming in to be treated for a TMJ condition, click here; for a sleep condition, click here. Download the paperwork, fill it out with a black or dark blue pen, and bring it with you to your first appointment.

At your first appointment, you’ll have a consultation where we gather information about your symptoms, the history of your disorder, your medical and dental history, and any previous treatment that you may have had.

After that, we perform a comprehensive initial exam that includes palpating the head and neck muscles, testing range of jaw motion, recording irregularities in jaw movement, observing posture, and doing a neurologic screening. We often find it necessary to take X-rays, which are done with our state-of-the-art, low-radiation cone beam computerized tomographic machine, which produces three-dimensional color images of the head and neck.

One we have a definitive diagnosis of your condition, we can recommend the best course of treatment for you.

Insurance Coverage

We are participating providers for some of the major insurers including BCBS and HealthPartners, and we also work with many other insurances. When calling to check your benefits, you may need to give them our National Provider Identifier (NPI) number as follows:
